How Do I Schedule a Virtual Consultation?

Whether you are a tech novice or expert, we have made the virtual consultation process easy for our Chicagoland patients. Simply follow these five steps to get virtual care from Dr. Chander.

Fill Out Our Virtual Consultation Form

Fill out our virtual consultation form and indicate when would be a good time for us to schedule your appointment.

Download and Setup the EyecareLive App

Go to the app store on your iPhone or Google Play store on your Android, download the app, and then create an account by providing your email address and selecting a password. You can then go to the menu and select Find Doctor. Enter Primary Eye Care Associates in the search bar and then select us as your provider.

Connect with Our Patient Coordinator

One of our patient coordinators will be in touch with you to confirm your virtual consultation time and date. We can also assist you in setting up the secure EyecareLive app.

Fill Out Any Necessary Forms

We will send you any necessary forms prior to your appointment at our Chicago practice. Please fill these out and submit them back to us before your scheduled virtual consult.
